It helps our home care providers learn and understand the specific needs of the client, so we can create a custom home health care plan to follow in the future. We can conduct a nursing assessment at a hospital, nursing home facility, or in the client’s home. 

The assessment serves to collect information about the client’s physical, psychological, and sociological status for future home health care. A licensed registered nurse from our team will conduct the nursing assessment.

Our home care agency may ask for certain information during the nursing assessment, such as a patient history. The home care provider will need to know about any conditions, symptoms, health histories, and medications. 

Our home health agency will conduct an interview to obtain the information we need. We may also give the client a physical examination or focused assessment on mental state, chronic pain, etc. Our licensed nurses are friendly, experienced, and committed to making the assessment process as easy as possible prior to beginning at-home care services.
